Your first visit to Harvard Dental Group establishes a vital foundation for our relationship with you.  During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You’ll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There’s also background information about our committed staff and our first visit procedures. You can even save some time at your first visit by printing out and completing the New Patient forms in advance of your appointment.

What To Expect
Being well-prepared for your appointment will ensure the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this website and take a few minutes to review our staff page and familiarize yourself with our doctors. We look forward to meeting you at your first visit to our practice.

Appointments
We know you have many choices when choosing a dentist in Douglas County so we have made requesting an appointment a simple process via our website. Our appointment times are reserved especially for you. Any change in your appointment affects many patients. We ask that you notify our office at least 24 hours in advance if appointment changes are necessary. This will allow us to accommodate patients who may be waiting for an appointment time. If you are a "Forever Whitening" patient, we ask that you provide 48 hours notice for all changes to appointments.

Insurance

Our practice currently accepts assignment for almost all insurance carriers. While many offices require full payment at the time of service, we ask only that you pay your estimated portion for treatment. As a courtesy and a benefit to our patients, we file your insurance claim and do our best to inform you if you will be responsible for any financial portion. Professional care is provided to you and not to an insurance company thus the insurance company is responsible to the patient, and the patient is responsible to the doctor.

Driving Directions

Directions:  Take exit 124 off Interstate 5 and proceed west approximately 0.6 miles to the corner of Fairhaven & Harvard. We’re on the south side of the street in the familiar “clock tower” building.  Look for us right across the street from the Umpqua Valley Arts Association, and between T-Mart and Tim’s Appliance.
