Appointments
We know you have many choices when choosing a dentist in Douglas County, so we have made requesting an appointment a simple process via our website. Our appointment times are reserved especially for you. Any change in your appointment affects many patients. We ask that you notify our office at least 24 hours in advance if appointment changes are necessary. This will allow us to accommodate patients who may be waiting for an appointment time. If you are a Forever Whitening patient, we ask that you provide 48 hours notice for all changes to appointments.

Insurance and Billing
Our practice currently accepts assignment for almost all insurance carriers. While many offices require full payment at the time of service, we ask only that you pay your estimated portion for treatment. As a courtesy and a benefit to our patients, we file your insurance claim and do our best to inform you if you will be responsible for any financial portion. Professional care is provided to you and not to an insurance company thus the insurance company is responsible to the patient, and the patient is responsible to the doctor.

Payment Options
In an effort to keep dental care affordable while maintaining a high level of professional care, we have established several payment options. A 10% cash discount is available to patients who do not have insurance and pay in full at time of service. We accept all major credit cards and can offer you interest-free financing through Care Credit. Care Credit is a healthcare credit card and can help you pay for treatments and procedures your insurance doesn't cover. With their instant approval process, it's easy to apply for CareCredit. You can apply either in our office, or by filling out an on-line application at www.carecredit.com. If you need assistance, please call us.
